HIPAA and PHI
In order to provide the Services to our Healthcare
Providers, Planet Doctor may gather certain demographic, health, and/or
health-related data about Users. This data may be classified as "protected
health information," or "PHI," and is subject to the Health
Insurance Portability and Accountability Act and its implementing regulations
(collectively, "HIPAA"). The circumstances are as follows: (i) Planet
Doctor is offering administrative, operational, and other services to a healthcare
provider that is a “Covered Entity” (as defined by HIPAA); and (ii) in order to
perform those services, Planet Doctor is acting as a “Business Associate” (as
defined by HIPAA) of the healthcare provider and receives identifiable
information about a user on behalf of the healthcare provider.
HIPAA limits the use and disclosure of PHI while
offering special protections for its security and privacy. To learn how your
PHI may be used and released, please read your health provider's Notice of
Privacy Practices. Any Personal Data (described below) that is not PHI is not
covered by HIPAA. PHI does not apply to personal information that a user gives
to Planet Doctor while Planet Doctor is not serving as a business associate.
Creating an account, searching for healthcare providers or available
appointments, completing general medical history forms that are not provided by
or required by a specific healthcare provider, posting reviews, providing
device/IP information, or providing Web Analytics information are just a few
examples of the various actions you may take (see below).
The categories of Personal
Data that we gather and have collected during the last twelve (12) months are
described in detail in the following subsections. Any information that
identifies or is related to a specific individual is considered "personal
data," which also includes information that is referred to as
"personal information" or "personally identifiable
information" in accordance with any applicable laws, rules, or regulations
pertaining to data privacy. These subsections also list the categories of third
parties with whom we disclose Personal Data, the source of the Personal Data
for each category, and our commercial or business reason for obtaining the
Personal Data. Further details about those sources and categories are provided
below.

Categories of Personal Data We Collect
Payment Information: We obtain payment-related information directly from
you, including information about the type of payment card you use, its last
four numbers, your billing contact information, and your billing email. For
transactions, this information is essential. We disclose this information to
our reliable service providers for business purposes, particularly Paypal,
Stripe, Inc. ("Paypal, Stripe"), our partner for payment processing.
Device/IP Information: When you use our services, automatically obtained
data about your device and IP is gathered. This contains the kind of device,
operating system, and browser being used, along with your IP address, device
ID, and domain server. In addition to third parties including ad networks,
analytics partners, service providers, and third-party business partners that
are accessed through the services, information is also collected directly from
you.
Web Analytics: We use web analytics to collect information about how
users interact with webpages, sources and webpages that we recommend them to,
non-identifiable request IDs, and other statistics pertaining to how your
browser or device interacts with our services. Ad networks, analytics partners,
service providers, and other third-party business partners that are accessed
through the services will receive this information, which is obtained from you,
for business purposes.
Geolocation Data: Your IP address is used to gather geolocation data
straight from you. Ad networks, analytics partners, service providers, and
other third-party business partners that are accessible through the services
are all given access to this information for business purposes.
Other Identifying
Information That You Voluntarily Choose to Provide: We obtain information directly from you that you
freely give us, such as passwords and personal data you enter in emails or

User Contact Data: We obtain your contact details directly from you,

User Demographic Data:
We obtain your demographic information

gender identity, age, date of birth, zip code, race, and sexual orientation.
Service providers, ad networks, healthcare providers, health information
exchanges, and others you approve, access, or authenticate are among the
businesses with which this information is shared for commercial purposes.
Medical Data: Direct collection is used to gather health-related
data, such as details about medical illnesses, healthcare providers visited,
visits' purposes and dates, and your own medical history. This information is
shared for business purposes with insurance companies, healthcare providers,
service providers, health information exchanges, and parties you authenticate,
approve, or permit.
Insurance Information: We obtain information about your insurance directly
from you, including the insurance carrier, insurance plan, member ID, group ID,
and payer ID. We exchange this information for business purposes with service
providers, hospitals, health information exchanges, and other partners you
permit, access, or authenticate.
Booking Appointment
Data: Information about scheduling an appointment,
including the date and time of the visit, the provider, the treatment, and if
you are a new patient, is gathered directly from you and other sources. This
data is shared with healthcare providers, analytics partners, service
providers, and health information exchanges for business purposes.
Social Network Data: Both you and third parties provide information about
your social networks, such as your email address, phone number, username, IP
address, and device ID. This information is shared for commercial purposes with
ad networks, service providers, and parties you verify, approve, or provide
access to.
Categories of Data
Considered “Sensitive” Under Certain Privacy Laws: Lastly, we recognize that certain data is sensitive in accordance with privacy
rules. This covers personal data including sexual orientation, health, and
account username and password. You may be confident that this private
information is treated with the highest care and shared only with reputable
service providers, insurers, healthcare providers, and health information
exchanges, as well as other parties you approve, access, or authenticate when
needed for business needs. We take the security of your data and privacy very
seriously.
Categories of Sources of Personal Data from You:

with direct interactions when you register for an account or use interactive
features like looking up healthcare providers or open appointments, or filling
out your medical history before seeing a healthcare provider. In addition,
adding data to the database involves filling out free-form text boxes,

correspondence with us become an invaluable source of information as well.
Moreover, cookies—which keep track of your online actions and
preferences—automatically gather personal data when you use the services.
Certain apps and software can be downloaded in order to receive data from your
computer, such as information about your current login status and whether you
are available for updates or alerts. Location and mobile device data may also
be collected if you use our mobile application or a browser that supports
location sharing. Outside of your direct encounters, contributions from other

From Third Parties:
In order to provide customer support and analyze your interactions with
the services, service providers are hired. They also play a key role in the
creation of user profiles and lead generation, which deepens our understanding
of user engagement.
Analytics partners work together to share information about website
traffic and service usage, which helps with service marketing and optimization.
Healthcare providers also play a significant role in this process,
exchanging pertinent information to make arranging appointments easier and

If you sign in with a third-party website or enter your credentials,
social networks come into play since data and content from those accounts could
be transferred to your account with us.
Advertising partners support marketing and promotional initiatives by
providing insights into user engagements with services, ads, or messages.

Healthcare Providers are the ones that receive Personal Data. For instance,
data is shared appropriately if a user fills out a Medical History Form ahead
of time and chooses to share it with the healthcare provider of their choice.
This sharing also applies to users who choose to use our services for analysis
and referrals regarding possible medical conditions or treatments, as well as
emergency scenarios.
Insurance providers acquire Personal Data in order to evaluate
eligibility, ascertain cost-sharing responsibilities, and request details about
benefit plans on behalf of consumers.
User information is sent to Health Information Exchanges and affiliated
organizations, like Regional Health Information Organizations, to improve the
safety and accessibility of data for healthcare professionals. The goals of

Parties You Authorize,

Professional License
Information: For commercial objectives, information
regarding healthcare professionals' specializations, certificates, and
educational background is obtained from you and shared with service providers.

Mobile Device Identifiers Planet Doctor uses mobile
device identifiers to get more data about the demographics and online habits of
its users. Mobile device identifiers are files kept on mobile devices that can

to collect Personal Data, including address, location, media access control,

industry consensus over what constitutes a Do Not Track signal, and not all
browsers offer a Do Not Track option. Please be aware that, similar to many
website operators, we do not currently implement "Do Not Track"
requests received from a browser due to these reasons and our usage of Cookies.

Information about Interest-Based Advertisements
Through the Services, we
might offer advertisements as well as permit third-party ad networks, such as
ad agencies, ad servers, ad technology providers, and research companies, to do

HIPAA and PHI
In order to provide the Services to our Healthcare
Providers, Planet Doctor may gather certain demographic, health, and/or
health-related data about Users. This data may be classified as "protected
health information," or "PHI," and is subject to the Health
Insurance Portability and Accountability Act and its implementing regulations
(collectively, "HIPAA"). The circumstances are as follows: (i) Planet
Doctor is offering administrative, operational, and other services to a healthcare
provider that is a “Covered Entity” (as defined by HIPAA); and (ii) in order to
perform those services, Planet Doctor is acting as a “Business Associate” (as
defined by HIPAA) of the healthcare provider and receives identifiable
information about a user on behalf of the healthcare provider.
HIPAA limits the use and disclosure of PHI while
offering special protections for its security and privacy. To learn how your
PHI may be used and released, please read your health provider's Notice of
Privacy Practices. Any Personal Data (described below) that is not PHI is not
covered by HIPAA. PHI does not apply to personal information that a user gives
to Planet Doctor while Planet Doctor is not serving as a business associate.

appointments, completing general medical history forms that are not provided by
or required by a specific healthcare provider, posting reviews, providing
device/IP information, or providing Web Analytics information are just a few
examples of the various actions you may take (see below).

four numbers, your billing contact information, and your billing email. For

Stripe, Inc. ("Paypal, Stripe"), our partner for payment processing.
Device/IP Information: When you use our services, automatically obtained
data about your device and IP is gathered. This contains the kind of device,
operating system, and browser being used, along with your IP address, device
ID, and domain server. In addition to third parties including ad networks,
analytics partners, service providers, and third-party business partners that
are accessed through the services, information is also collected directly from
you.
Web Analytics: We use web analytics to collect information about how
users interact with webpages, sources and webpages that we recommend them to,
non-identifiable request IDs, and other statistics pertaining to how your
browser or device interacts with our services. Ad networks, analytics partners,
service providers, and other third-party business partners that are accessed

straight from you. Ad networks, analytics partners, service providers, and
other third-party business partners that are accessible through the services

ad networks, service providers, and parties you verify, approve, or provide

In order to provide customer support and analyze your interactions with
the services, service providers are hired. They also play a key role in the
creation of user profiles and lead generation, which deepens our understanding
of user engagement.
Analytics partners work together to share information about website
traffic and service usage, which helps with service marketing and optimization.

taking part in our user study. Our assessment of the estimated value of your
data informs the cash incentive we provide. You might be requested to provide
personal information in surveys, questionnaires, or through your account if you
take part in the study. We will never sell or disclose the personal information
you provide to us for this research with outside parties; it will only be
utilized internally to enhance our offering.
Your involvement in the study gives us permission
